The Ronald McDonald Care Mobile will be at the school on October 5th and 6th , 2022. Please call to schedule an appointment if your student still needs the requirements listed below:
Physicals and Immunizations - Pre-K, Kindergarten, 6th grade, 9th grade and all new students to the district.
Immunization only - 12th grade

The care mobile provides free medical care to uninsured or underinsured children ages 0 to 18 years. This is a free and confidential service.
Medical visits will be available by appointment only. Please contact Sharon Hoover at 815-732-5300 Ext: 2222 or Sara Rowe Ext: 1122. Consent forms are available on the district website home page or can be picked up in the school offices.
