ISBE is requiring that school physicals for the 23-24 school year be completed on the following IL exam form https://trst.in/zi1txz Any other forms will not be accepted.

 

Please ensure your medical provider uses the appropriate form when completing your student's physical. Paper copies are available in the main office if your provider does not have the correct form.

 

Physical exams are required for incoming pre-schoolers, kindergarteners, 6th graders, 9th graders, and any students that are new to IL schools.

You are the best judge of your child’s health. However, we would like to emphasize that unnecessary absenteeism will only make your child fall behind in his/her classroom studies. Some children complain of not feeling well in the morning, but feel better after getting awake and getting fresh air and activity.

Please use the following guidelines to determine the need to keep your child home:

  •       Temperature 100 degrees or over. (Your child needs to be fever free for 24 hr. before returning to school)
  •       Diarrhea/vomiting (Can return to school if no incident of diarrhea/vomiting within 24 hours)
  •       Undiagnosed rash
  •       Any contagious condition
